If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We accept cash, checks, and all major credit cards. For patients needing more extensive dental work, we also offer third party financing through Care Credit. Third party financing must be approved before services are rendered. If you have insurance, we will collect any deductible and co-payment at the time of your appointment. A 1.5% finance charge will be applied to balance after 30 days.
Although our office as a courtesy will bill your insurance company for you, your insurance is a contract between you and your insurance. The office is not responsible for payment by your insurance company. You are still fully responsible for the charges for treatment rendered. Your insurance may not cover the services or may only partially cover them depending on your contract. Also, most all insurance plans have an annual maximum, which limits the amount payable during a fiscal year. Any estimate giving by the office is considered a guideline.
When we schedule your appointment, the time is reserved exclusively for you. Failure to notify us of your inability to keep an appointment, prevents another patient in need of dentistry is unable to receive treament. We request that you give us at least 24 hours notice when you realize that you cannot keep your appointment. When the request notice is not given, a fee of $48 per half hour scheduled will be charged.
